Biventricular Stress-induced Cardiomyopathy and Cardiogenic Shock Secondary to Rheumatoid Arthritis Flare

چکیده
Stress-induced cardiomyopathy has a wide variety of clinical presentations ranging from angina symptoms to cardiogenic shock, as well as an array of echocardiographic findings and etiologies. We present a unique case of severe biventricular dilation with cardiogenic shock but rapid recovery within 96 hrs. Severe biventricular stressinduced cardiomyopathy secondary to acute rheumatoid arthritis (RA) flare was diagnosed and the patient later regained full recovery after circulatory support and RA treatment.

Background: Heat-shock proteins are part of a strictly controlled biological system that allows organisms to respond to environmental stresses. Different proinflammatory cytokines are present in the synovial tissue of rheumatoid arthritis patients. Such tissues respond to stress and induce heat-shock proteins. In addition, synovial cells are exposed to mechanical stress caused by joint motion. ...
متن کاملCardiogenic Shock due to Psychosis-Induced Inverted Takotsubo Cardiomyopathy Bridged-to-Recovery with a Percutaneous Left Ventricular Assist Device
Inverted Takotsubo cardiomyopathy, a less common variant in the spectrum of stress-induced cardiomyopathy, is increasingly being reported. This report describes an acute psychiatric illness leading to the onset of this syndrome. The patient presented here developed cardiogenic shock but successfully recovered with the use of a percutaneous left ventricular assist device.
متن کاملLevosimendan: the inotrope of choice in cardiogenic shock secondary to takotsubo cardiomyopathy?
Takotsubo cardiomyopathy (TC) has become an increasingly recognised entity in Western literature since its initial reporting in Japan. The pathogenesis underlying the myocardial stunning and systolic dysfunction is thought to be induced by elevated systemic levels of catecholamines and neuropeptides. Whilst the majority of patients are haemodynamically stable, a small proportion can develop car...
متن کاملStress-Induced Cardiomyopathy Presenting as Shock.
Stress-induced cardiomyopathy has become a more recognized and reported entity. It can be caused by emotional or physical stress, which causes excessive catecholamine release. Typically, the clinical course is benign with conservative treatment being effective. However, stress-induced cardiomyopathy can be fatal. A 41-year-old female presented with cardiogenic shock followed by sudden back pain...
متن کاملManagement of an acute catecholamine-induced cardiomyopathy and circulatory collapse: a multidisciplinary approach
A phaeochromocytoma (PC) is a rare, catecholamine-secreting neuroendocrine tumour arising from the adrenal medulla. Presenting symptoms of this rare tumour are highly variable but life-threatening multiorgan dysfunction can occur secondary to catecholamine-induced hypertension or hypotension and subsequent cardiovascular collapse. High levels of circulating catecholamines can induce an acute st...
